Patient Satisfaction Survey
Students between the ages of 12 and 19 who visit NKN Student Health & Wellness Center may be asked to complete an anonymous survey after their visit. The survey asks about the student’s satisfaction and experience at the health center, as well as some general questions about their physical and mental health status. Students can refuse to take the survey, and this will not affect their ability to get care at NKN Student Health & Wellness Center. To see a copy of the survey or if you have questions about the survey, please contact the School Based Health Center State Program Office at sbhc.program@dhsoha.state.or.us
What is the difference between services provided by NKN Student Health & Wellness Center and the NKN School Nurse?
The school-based health center care team performs routine physical exams, sports physicals, age-appropriate reproductive health services, well child care, diagnosis and treatment of acute and chronic illnesses, and other health and wellness-related matters for which the student seeks care at NKN Student Health & Wellness Center.
The school nurse provides nursing services to students during the school day, including health care planning while at school, daily medications, and coordination of available resources for students and families.
The care team at NKN Student Health and Wellness Center cannot share any personal health information about a student with the school nurse unless a signed Release of Verbal Medical Information or Authorization to Release Medical Records is on file.
Likewise, the NKN School Nurse cannot share any personal health information with NKN Student Health and Wellness Center about a student unless they are authorized to do so.
Student Health Advisory Committee
We are committed to having student voices involved and are working to form an advisory committee with students from NKN Middle School and High School.
Health & Wellness Center Community Advisory Committee
The Advisory Board strives to ensure that the Neah-Kah-Nie Health Center is designed and developed to meet the needs of the community. The group meets two or three times a year, or more frequently if necessary.
